Yes, yes. Usually a note too crumpled to be returned. It just gets narky and eats it.
On the reverse of that, I've been sitting next to a Vortex once, minding my own business and the note acceptor just whirred into life, doing that 'back and forward' activity, and after about 30 seconds or so, spewed out a £20.
Of course, I 'acceptored' it and thought that's the best profit I've made from a machine! £0 in for £20 out.
Agree though that noteys can really ruin a session.
"And do you ever contradict yourself, Minister?" "Well, yes and no..."
Had a machine on test before. Cycle through 10p payout, 50p payout, 20p payout, jackpot music. Took about an hour to empty, but everyone kept looking at me with jp music every minute.

Most annoying thing for me was doing the WIYB in Leeds wetherspoons which was right next to the bog. Every time someone went in and left the door to close on its own, it creek for 5 or six seconds before closing with a thump. Sort of: Eeeeeeeeeeeeek CLUMP! CLUMP! Sounded like Sweep from the Sooty Show being clubbed to death!
