Okay, David Prowse played him in the tall black cloak and coal scuttle, which was dubbed over by James Earl Jones. And a bloke called Sebastian Shaw played him unmasked and as a force ghost at the end of ROTJ.

I expect there were also various stunt doubles, but I've no idea who they were.

there were apparently 4 not including stunt doubles:

1) Mr Prowse was the body

2) Sebastian Shaw was the face

3) JEJ was the voice

4) Another voice actor, whos name i couldnt find, who did all the heavy breathing!
