MJD some pubs are test sites and probably why you always see new machines coming and going. My local used to be the same I dont think there has been a machiine lats longer then 5 months in there

I think a S16 only holds £600 in coin (May be more with an upgrade)

Most people play with notes so the coins will nearly always run out. I think I have only seen a dozen or so £500 wins pay out in full

I doubt they will widely use such systems as they will eventually be hacked and this to me says no more refill key. All you would need is a little PDA telling you how much is in the machine and if somebody has just emptied it.
